How To Submit A GRAMA Request

 

 

Wasatch Fire District adheres to the Government Records Access and Management Act (GRAMA). GRAMA is the records law for the state of Utah. Wasatch Fire also complies with the Health Insurance Portability and Accountability Act (HIPPA). We will respond to your request as soon as reasonably possible, but no later than 10 business days after receiving the written request. To submit a request please see the directions below.

 

Fire/Hazmat Record Requests:

Anyone requesting fire reports or information pertaining to a hazmat incident, please fill out the application form and email to [email protected].

 

Medical Record Requests:

Anyone requesting a patient care report or patient bill must submit the request to JCI Billing. Please fill out the same application form along with a release signed by the patient stating that protected information may be released and email to [email protected].  For further information their contact information is listed below:
